Disability Assessment

It is estimated that approximately 13.3% of residents 50 years old and above (1 in 10) suffer from some form of disability. With our aging population, the burden of disability is expected to rise. In Singapore, there are various National Disability Schemes to provide financial support for people who suffer from disability due to old age, illness or injuries. 

 

Our doctors are MOH-accredited assessor and have many years of experience performing house visit disability assessment to help patients with their application of National Disability Schemes (Eldershield, Careshield and various other schemes). We understand the common issues that patients and their family members face and are always ready to guide them through the process within our capacity.

  • What are the various National Disability Schemes?
    The National Disability Schemes are classified according to the level of disability - Mild, Moderate, Severe. MILD DISABILITY - Migrant Domestic Worker Levy Concession MODERATE DISABILITY - Pioneer Generation Disability Assistance Scheme (PioneerDAS) - Home Caregiving Grant (HCG) SEVERE DISABILITY - Interim Disability Assistance Programme for the Elderly (IDAPE) - Elderfund - Eldershield - Careshield - Medisave Care
  • How do I know which Schemes I can apply for?
    Agency for Integrated Care (AIC) is the agency that governs all the National Disability Schemes.   • How do I apply for Eldershield, Careshield or other schemes?
    1) Application via online using Singpass - This is the best and fastest method if you have access to your own or your loved ones Singpass. - You can apply through the following link: https://efinance.aic.sg/ 2) Using Hardcopy forms - You may acquire and submit hardcopy application forms from AIC offices - Location of AIC office : https://www.aic.sg/about-us/aic-link-locations/
  • Can any doctor or my regular GP do the assessment for me?
    Your doctor or regular GP needs to be MOH-accredited assessor to be able to assist you with the assessment and submission of medical report for application of severe disability schemes.
  • Who are MOH Accredited Disability Assessors?
    All MOH-accredited assessors have undergone a rigorous training programme to learn how to assess a Patient’s level of disability. Through the training, assessors are equipped to assess an individual’s ability to perform the six ADLs, including specific components of each ADL to take into account, what factors to consider when an individual is cognitively impaired, and what should be done if any individual’s functional ability fluctuates over time. As this is a complex process, the training and accreditation is necessary to ensure that disability assessments are conducted with a high degree of rigour and consistency. Source: MOH/AIC
  • What are the components for Disability Assessment?
    Disability Assessment is based on the 6 Activities of Daily Activities and the level of assistance required for each activities. To qualify for severe disability schemes, 3 out of the 6 Activities must require maximum level of assistance. Source: AIC
  • How much does it cost to do a disability assessment?
    The price of severe disability assessment is standardized by AIC. The fees are as follows, if: You visit an assessor’s Clinic: $100 An assessor visits your house: $250 For Careshield, the assessment fee for the first disability assessment will be waived, regardless of whether the care recipient is assessed to be severely disabled, or whether the claim is successful. If this is not the care recipient’s first time being assessed for CareShield Life claims, assessment fee from you. If the care recipient is assessed to be severely disabled, the full assessment fee will be reimbursed to you. For all other schemes, assessment fee will be collected. If the care recipient is assessed to be severely disabled, the full assessment fee will be reimbursed to you.
  • I have a Functional Assessment Report (FAR). Can I use it to apply Eldershield or Careshield?
    You will not be able to use Functional Assessment Report(FAR) to apply for Eldershield or Careshield. FAR is a simple, less detailed functional assessment and can only be used to apply for mild or moderate disability scheme. For severe disability schemes like Eldershield and Careshield, a detailed Assessor statement report by MOH-accredited assessor will be required.
  • When faced with disability, what other things should I plan or apply for?
    The following can be performed during the house visit for disability assessment with additional charges: (It is important to consider having the following done in the same setting if required so that you can save on the cost for another house visit) Mental Capacity assessment LPA LPA activation assessment (if already has LPA and lose mental capacity) Advance Medical Directive Handicap Car Park Label Application Mobility Report form Handicap relief IRAS Form - for tax relief Insurance form Other Important things to consider: 1) Advance Care Planning (ACP) - to look for an ACP facilitator : Click here 2) Will - performed by lawyer
